I have a right to rescind (stop) my enrollment within 10 days after my utility has accepted the order to switch suppliers by contacting ComEd directly or by contacting Dynegy at 833-265-6999 or by emailing to [email protected].
I authorize Dynegy to contact me at the phone number I provided in this enrollment. Contacts by Dynegy may be made using automatic dialers, text messaging, or prerecorded messages. I understand that my consent is not a condition of purchase
PowerSetter will send you an email notification in advance of your contract expiration, reminding you of your contract terms and renewal options.
I verify that all information is correct, and that I have authorization to change this account. I understand the contract, and is voluntarily authorizing the enrollment, renewal or switch (as applicable) to the supplier shown on this enrollment form. I understand that electronic acceptance of the sales agreement and terms of service is an agreement to initiate service and begin enrollment. I understand that energy supply will be provided by Think Energy Test and energy delivery shall continue to be provided by ComEd. ComEd will also be available to respond to leaks or other emergencies should they occur.
I understand that I have an affirmative obligation to provide the supplier with any change in e-mail address, and/or with any withdrawal of consent for the electronic retention of contracts and/or other customer information.
Letter of Authorization
I, the authorized signatory above, affirm my understanding of the following:
1) That I am authorized to make changes to my energy account and I agree to change my electric and/or natural gas provider from my current provider to Dynegy.
2) That the Terms of Service and nature of service, including the rate to be provided to me have been clearly and conspicuously disclosed to me in writing.
3) That my correct name and billing address is listed above.
4) That I will receive a copy of my Terms of Service and the Rate Plan Summary and have been advised of my right to rescind.
5) That I understand that only one generations company, aggregator or supplier may be designated as the consumer's competitive supplier.
6) That I understand any electric and/or natural gas service provider selection that I choose may involve a charge to me for changing my electric and/or natural gas service provider.
