You have the right to cancel this service agreement until midnight of the third business day after the date that you receive this agreement by contacting Constellation directly at 800-785-4373 or emailing them at feedback@constellation.com.

I verify that all information is correct, and that I am the customer of record, or have the authority to enter into a contract for this account. I understand the contract, and I am voluntarily authorizing the enrollment, renewal or switch (as applicable) to the supplier shown on this enrollment form. I understand that electronic acceptance of the sales agreement and terms of service is an agreement to initiate service and begin enrollment. I understand that energy supply will be provided by Constellation and energy delivery shall continue to be provided by Eversource. Eversource will respond to leaks or other emergencies should they occur.
