You have the right to cancel this service agreement until midnight of the third business day after the date that you receive this agreement by contacting Constellation directly at 800-785-4373 or emailing them at [email protected].

I verify that all information is correct, and that I am the customer of record, or have the authority to enter into a contract for this account. I understand the contract, and I am voluntarily authorizing the enrollment, renewal or switch (as applicable) to the supplier shown on this enrollment form. I understand that electronic acceptance of the sales agreement and terms of service is an agreement to initiate service and begin enrollment. I understand that energy supply will be provided by Constellation and energy delivery shall continue to be provided by United Illuminating. United Illuminating will respond to leaks or other emergencies should they occur.
